Summary of Insurance Coverage
Underwritten by Monumental Life Insurance Company, Cedar Rapids, Iowa

1. Guaranteed Acceptance by Monumental Life: Every Bank of America customer, age 18 and older. who enrolls for Group Accidental Death Insurance Coverage will be accepted by Monumental Life Insurance Company - regardless of health or occupation (see Exclusions). No physical examination is necessary and there are no medical questions to answer.
2. 3D-Day (No-Cost-To-You) Period: As a valued Bank of America customer age 18 and older, you are eligible to receive the first 30 days of coverage at no cost to you. The monthly cost for your first 30 days of coverage will be fully paid for you by Bank of America, N.A. To begin your coverage, complete and mail the enclosed Activation Form.
3. Group Rates: After your 30-day no-cost-to-you period, you can continue this coverage at group rates. Coverage, under the Family Plan, is available for your spouse and any unmarried dependent children under age 19 (25 if a full-time student in an accredited institution of higher education); 26 if in UT. Coverage amounts decrease to 50% at age 70, and to 25% at age 75.
4. Family Plan: If you select the Family Plan, your spouse will be insured for 50% of your insurance coverage amount and each eligible child will be insured for 10% of that amount. If you have no spouse, Y0lll' children will each be insured for 15% of your insurance coverage amount. If you have no eligible children, your spouse's coverage will be equal to 60% of your insurance coverage amount. You may name any beneficiary on your Activation Form. If you select the Family Plan, you are automatically your spouse's and dependents' beneficiary.
5. Also Included With Your Coverage:
Education Assistance Benefit. Under the Family Plan, this benefit provides an additional 2% of the amount of insurance in force at the date of death of the primary insured for each eligible dependent child who is enrolled on the date of the accident as a full-time student in an institution of higher education beyond the 12th grade level. This additional 2% benefit will be paid each year up to a maximum of four years. If you have no eligible children who qualify for this benefit, Monumental Life will pay your beneficiary an additional 2% of your amount of insurance in force at your date of death.
Paralysis Benefit. This benefit pays 100% of the benefit amount if the insured is in a covered accident that results in Quadriplegia (total paralysis of both upper and lower limbs) and 50% of the benefit amount if the insured is in a covered accident that results in Paraplegia (total paralysis of both lower limbs) or Hemiplegia (total paralysis of upper and lower limbs on one side of the body). The paralysis must occur within 31 days from the date of the accident. The paralysis also must be continuous for at least 365 days for the benefit to be payable.
Dependent Child Day Care Benefit. Under the Family Plan, this benefit pays 2% of the face amount of the insurance in force per year for up to two years for day care for dependent children under the age of 14, should the insured die as a result of a covered accident. The Dependent Child must be attending a day care center on the date of the accident causing the injury or be enrolled within 90 consecutive days from the date of the accident causing the injury. If no children qualify, the death benefit pays a one-time lump sum amount of 2% of the insurance in force.
Special Training Benefit. This benefit provides the spouse of the insured an additional 2% of the benefit amount, not to exceed the cost of enrollment, if the spouse is enrolled on the date of the accident causing the injury or enrolls within 12 months in any professional or trades training program or educational degree program. Family coverage must be in force.
6. Renewal of Coverage: The insurance coverage you select is renewable and cannot be cancelled as long as your premiums are paid when due, the Master Policy remains in effect and you remain a Bank of America customer. Dependents are covered for as long as they remain eligible. Any premium increase will apply to the entire master group policy.
7. Exclusions: Benefits will be paid for any accidental death except when caused by, resulting from or contributed to by: suicide, attempted suicide or intentionally self-inflicted injury while sane or insane (in MO and CO while sane); any act of war (declared or undeclared); full-time military service; participating in a riot, committing an assault or felony; sickness and its medical or surgical treatment, including diagnosis; bacterial infections except through a wound accidentally sustained; operating or riding in any kind of aircraft, except as a tare-paying passenger on a regularly scheduled commercial flight or as a passenger in a transport plane operated by the Air Mobility Command (AMC) of the United States of America; alcohol intoxication, as defined in the State where the accident occurred; taking of any drug, medication, narcotic or hallucinogen, except as prescribed by a Physician; taking alcohol in combination with any medication or sedative; gas inhalation or poison voluntarily taken or administered; riding or driving as a professional in any kind of race for prize money or profit.

8. About The Underwriter: This plan is underwritten by Monumental Life Insurance Company of Cedar Rapids, Iowa, an AEGON Company. Monumental Life Insurance Company is rated "A+" (Superior) for financial strength, second highest of 16 categories, by the A.M. Best Company as of June 29, 2010.
9. Coverage Activation: To activate your insurance coverage, just select the amount of coverage you wish then complete, sign and return the simple Activation Form. Be sure to check the box next to Family Plan to cover your family. Send no money. Unless you call to cancel your coverage, starting 30 days after the Effective Date, your premiums will be automatically deducted from your Bank of America checking account each month. A 75¢ administrative fee will be added by the Plan Administrator for each monthly automatic account billing. Your insurance will become effective on the Effective Date as stated on your Certificate of Insurance.
